We are at 1200 Park Avenue, Suite 400, on the Upper East Side. The 86th Street stop on the 4, 5 and 6 lines is the nearest subway.
02How should I prepare for a first visit?
Arrive about ten minutes early with your insurance card if you have one, a photo ID and a list of the medicines you take. Bring your questions.
03How do I pay?
How fees work explains insurance, paying yourself and receipts. We give a written estimate before any treatment. Financing is offered by third-party lenders. Approval and terms are set by the lender.
04Do I fill in health forms on this website?
No. The contact form asks for no medical details. Health history goes on the forms in the practice's own secure portal.
